Global brain connectivity in cognitive impairment after stroke.
Previous research has shown reduced connectivity in patients with cognitive impairment after stroke. However, it is unknown whether stroke patients with cognitive impairment also exhibit altered connectivities of the whole brain, and if this is associated with severity. The present study aimed to evaluate whole brain connectivity in patients with cognitive impairment and mild cognitive impairment (MCI) after stroke. We investigated global and local brain networks in 45 stroke patients with cognitive impairment and 35 healthy controls (HCs) using resting-state functional magnetic resonance imaging (fMRI). Brain regions were classified into seven functional networks based on a previously reported automated anatomical labeling atlas, and diffusion tensor imaging was used to compute fractional anisotropy (FA) within the whole brain. Compared with HCs, patients with cognitive impairment showed altered regional global and local network connectivities. No relationship was observed between cognitive impairment and regional FA in patients. However, global network FA in left dorsolateral prefrontal cortex (DLPFC) was significantly correlated with the Mini-Mental Status Examination (MMSE) score in patients with cognitive impairment. Furthermore, the connections between the left DLPFC and precuneus were positively correlated with the MMSE score in patients with cognitive impairment, whereas the connections between right DLPFC and precuneus were positively correlated with the MMSE score in MCI patients. We conclude that patients with cognitive impairment after stroke exhibited altered brain functional connectivity. Left ventricular dimensions and volumes were measured by M-mode and two-dimensional echocardiography in 105 normal adolescents, ages 10 to 19 years. The percent coefficient of variation of interstudy variability was determined as 10.6% for left ventricular end-diastolic volume and 19.6% for left ventricular end-systolic volume. The Doppler sample volume was placed in the left ventricular cavity in 22 of 24 patients and in the left atrial cavity in two patients. The M-mode echocardiographic measurements were not significantly different in these patients. Thus, M-mode echocardiography is well suited for the estimation of left ventricular dimensions and volumes and for the detection of left ventricular disease in normal adolescents.Q:
