ABSTRACTThe purpose of this study was to report a new epidemiological study of the prevalence and incidence of dermatomyositis (DM) in the adult population of a Southeast region of Brazil. This cross-sectional epidemiological study was conducted from June 2012 to June 2013 in Pelotas, RS, Brazil. Adult patients in both genders diagnosed with DM were included in this study. The diagnosis of DM was done by a dermatologist using the algorithm proposed by Bohan and Peter. A patient was diagnosed of DM if he or she presented with muscle weakness, typical skin lesions, and positive autoantibodies. From a sample of 1,701 individuals, 80 cases of DM were detected, resulting in a standardized prevalence of 4.8%. The mean age of the patients was 53 years. The female/male ratio was 2.9. No significant differences were found in the prevalence of DM between sexes. There was a positive association between DM and poor education and between DM and a low income. The prevalence of DM in the adult population of the city of Pelotas was relatively low, which supports the idea that in low prevalence areas there is a greater chance of detecting the real distribution of the disease.
Introduction
Dermatomyositis (DM) is a rare subgroup of idiopathic inflammatory myopathy (IIM). The disease is characterized by typical cutaneous and muscle manifestations, and it has an association with different autoimmune diseases, such as Sjögren’s syndrome, polymyositis and rheumatoid arthritis. Approximately 3-5% of DM patients develop lung fibrosis.1,2 The disease was first described by Bohan and Peter in 1969.3 It is characterized by cutaneous findings of heliotrope rash and Gottron papules, and it is clinically distinguished from the classic polymyositis. The dermatologists need to be more attentive to the diagnosis and differentiate DM from other forms of inflammatory dermatoses, such as dermatomyositis sine scleroderma, because these patients require different treatment regimens.1,4,5 Dermatologists must search for relevant etiological aspects, such as infection, metabolic and malignant
